چکیده

Healthcare applications are complex in the way data and schemas are organised in their internal systems. Widely deployed healthcare standards like Health Level Seven (HL7) V2 are designed using flexible schemas which allow several choices when constructing clinical messages. The recently emerged HL7 V3 has a centrally consistent information model that controls terminologies and concepts shared by V3 applications. V3 information models are arranged in several layers (abstract to concrete layers). V2 and V3 systems raise interoperability challenges: firstly, how to exchange clinical messages between V2 and V3 applications, and secondly, how to integrate globally defined clinical concepts with locally constructed concepts. The use of ontologies for interoperable healthcare applications has been advocated by domain and knowledge representation specialists. This paper addresses two main areas of an ontology-based integration framework: (1) an ontology building methodology for the HL7 standard where ontologies are developed in separated global and local layers; and (2) aligning V2 and V3 ontologies. We propose solutions that: (1) provide a semi-automatic mechanism to build HL7 ontologies; (2) provide a semi-automatic mechanism to align HL7 ontologies and transform underlying clinical messages. The proposed methodology has developed HL7 ontologies of 300 concepts in average for each version. These ontologies and their alignments are deployed and evaluated under a semantically-enabled healthcare integration framework.

Electronic Healthcare Record and Clinical Research in Cardiovascular Radiology. HL7 CDA and CDISC ODM Interoperability

Integrating clinical research data entry with patient care data entry is a challenging issue. At the G. Pompidou European Hospital (HEGP), cardiovascular radiology reports are captured twice, first in the Electronic Health Record (EHR) and then in a national clinical research server. Informatics standards are different for EHR (HL7 CDA) and clinical research (CDISC ODM). The objective of this w...

Design and implementation of Converter V2 and V3 for HL7 messages

HL7 appears in two series of versions; version 2.x (V2) and version 3.0 (V3). Both versions are not compatible with each other. V2 is a market success but not fulfilling the emerging requirements of the world. V3 has the fame to claim as global and futuristic standard. In America 93% hospital are using applications that are developed according to HL7 version 2.x specification. To enhance the ad...

Architectural approaches for HL7-based health information systems implementation.

OBJECTIVE Information systems integration is hard, especially when semantic and business process interoperability requirements need to be met. To succeed, a unified methodology, approaching different aspects of systems architecture such as business, information, computational, engineering and technology viewpoints, has to be considered. The paper contributes with an analysis and demonstration o...

Towards Automatic HL7-RIM and Relational Database Mapping

Health Level Seven (HL7) is an emerging standard in the healthcare industry. HL7 version 3.0 (v3) targets to achieve semantic interoperability among the healthcare systems through HL7 Reference Information Model (RIM). In any healthcare system, information is ultimately stored in the database, which requires mapping of database to HL7 RIM. A key bottleneck in building HL7 systems is the laborio...

Information Persistence Services Designed to Support Home Care

BACKGROUND Due to the challenges faced by health and social care systems, in particular those related to actual demographic trends, home care emerges as a potentially cost-effective solution to answer the needs of citizens, and to allow the reallocation of resources to alternatives to hospitalization or institutionalization. OBJECTIVE Home care services require cooperation between different a...
